Ralph Choate is a Medicaid operations and transformation executive with more than two decades of Medicaid leadership and more than three decades of operational experience. His work spans eligibility accuracy, PERM and MEQC, program integrity, regulatory performance, and large-scale transformation. He has led statewide Medicaid operations serving more than 1.7 million members and administering more than $20 billion annually. As founder, editor, and principal author of PERM Insights & Perspectives, he brings that experience to independent analysis of Medicaid policy, operational accountability, and fiscal exposure.

PERM Insights & Perspectives is an independent publication and developing research program examining how Medicaid policy becomes operational practice—and how that practice affects eligibility accuracy, accountability, payment-error measurement, and fiscal exposure.
Our work connects three functions. Articles explain developments, examine operational challenges, and offer informed perspectives. Research investigates defined questions through documented evidence and methods, with explicit limitations and review status. Tools translate evidence and operational experience into practical frameworks, checklists, and resources.
These functions share a mission while maintaining distinct publication standards. Editorial perspectives may raise questions for research; research findings may strengthen, refine, or challenge those perspectives. Practical tools draw on both, with their evidence and intended use made clear.
Our purpose is to help the Medicaid community understand risk, examine assumptions, and improve operational decisions.

Research publications identify their methods, sources, limitations, version, and review status. Research in progress is labeled accordingly, and material corrections and revisions are documented.
